New Patient Forms
Thank you for choosing our wonderful practice to serve you and your family. In order to better serve you, all new patients are required to complete the online new patient forms prior to scheduling your first appointment. New patients cannot schedule an online appointment, as we must verify all your information before making you an appointment. All new patients need to click on the section “NEW PATIENT FORMS” and completely fill out all the forms. Please be sure to hit “submit” at the bottom of the forms or else they will not save and you will have to redo them. Once you have submitted your new patient forms, we will now have your information at the office, and you may call to schedule your new patient appointment. Please do not just call the office to schedule a new patient appointment, as we will just have to direct you to our website to fill out these forms first. These forms may be filled out from any type of computer or your smart cell phone.
These forms allow us to check all your information prior to your visit to verify that your child has no potential health problems that need to be discussed prior to their dental visit. As a courtesy to you, it also allows us to verify your dental benefits plan (dental insurance) to make sure there are no problems or hold ups when you arrive for your child’s appointment. Without having the proper health and insurance information prior to your appointment, it could result in extra time when you check in, delayed appointments, and we may have to reschedule your appointment. Nobody wants that to happen.
It is also very important that you make your new patient appointment, or we may not be able to reschedule you. As a parent himself, Dr. Nathan understands that life happens and schedules change. However, if you just don’t show up or don’t even call to let us know you are not coming and don’t have a valid reason for missing, we may not be able to reschedule you and you may have to put down a deposit to reschedule. Let’s please not let that happen.
